The Grayson County Health Department has released its August restaurant inspection results.

The rating score of the establishment is determined by subtracting weighted point values of noted violations from 100 (one hundred). Unannounced inspections observe the overall operation including food source and protection; personnel; food equipment and utensils; maintenance of water and sewage systems; garbage and refuse disposal; insect, rodent and animal control; and storage of toxic items, among others.

Critical violations, four and five points in weight, are items which if uncorrected carry the potential to be an imminent threat to the public’s health. These items must be corrected in a time designated by the health department not to exceed 10 days.

There were several outstanding scores in August, including Dinner Bell (99 & 100) Mr. Gatti’s (99), Dairy Barn (99), Ready Mart (98), Kay’s Café (98), and 100s by the Leitchfield Aquatic Center Concession, Leitchfield Sports Complex Concession and Leitchfield Softball Concession.

There were also poor scores recorded, with the worst being Barrett’s Chevron, which recorded an 82 food score with 14 violations, including food not in good condition, milk out of date, chopped bacon out of date, and mold like substance/debris in ice machine.

Wendy’s: Food score 91 — Violations: No person in charge/food protection manager present at time of inspection, hand wash facilities not properly supplied (no hand towels), thermometers not preset in multiple coolers and freezers, stock (food) stored on floor in freezer, clean utensils not properly stored, debris in bottom of coolers, freezers & on sides/front of multiple equipment, back door in poor repair-can see daylight in multiple areas, toilet seat in bathroom in poor repair.

Grayson County Senior Center: Food score 94 — Violations: Handwashing facilities not supplied (hand sink in kitchen & women’s restroom), missing date marking in multiple items in refrigerator, contamination not prevented during storage, mops not properly stored, single use single service items not properly stored.

El Patron: Food score 96 — Violations: No approved thaw method (immediately corrected), wiping cloths not in sanitizer buckets, unapproved sanitizer used & mops not hung dry, toilet facility missing covered trash can.

Subway: Food score 97 — Violations: Hair restraints not worn by all handling food, non-food contact surfaces with debris, condensation on floor in freezer.

Barrett’s Chevron: Food score 82; Retail score 91 — Food violations: No person in charge present at time of inspection, food not in good condition, safe unadulterated, milk out of date, chopped bacon out of date (voluntarily discarded), mold like substance/debris in ice machine, food not at proper hot holding temperature (voluntarily discarded), multiple items in refrigerator without date marking, thermometers missing or not working in multiple freezers, in use wiping cloths not properly stored, single use/single service article not properly stored (on floor), garbage area with debris scattered around area, lids open, outside opening not closed or screened to prevent entrance of insects and rodents. Retail violations: Over the counter medicine expired, food contact surface-drink machines with mold like substance/debris in ice dispenser portion, bulk bagged ice for sale not properly labeled, single use/single service article not properly stored, garbage area with debris scattered around area, lids open, outside opening not closed/screened to prevent entrance of insects and rodents.

Dairy Queen: Food score 94 — Violations: Food with out-of-date date marking-multiple items, toxic substances not properly labeled, mops not stored properly, sanitizer buckets too weak (immediately corrected), free standing fans in food areas with debris, condensation in freezer on floor and around fans.

Mr. Gatti’s: Food score 99 — Violations: Free standing fans with build-up of debris in food area.

Huddle House: Food score 90 — Violations: Hand washing facilities not adequately supplied, date marking missing from items in refrigerators, food product stored on floor, in use utensils not stored properly, ware washing facilities not at proper temperature, no test strips present, plumbing not maintained at hand washing sink in grill/food prep area, toilet facilities not maintained (no covered receptacle in women’s bathroom, ceiling in poor repair, tiles missing and some with damage).

Kayla’s Corner Café: Food Score 97 — Violations: Wiping cloths not properly stored, in use utensils not properly stored, single use/single service items not properly stored.

Papa John’s: Food score 95 — Violations: Hand washing facilities not properly supplied and accessible, missing date marking multiple items in refrigerator/cooler, mops not properly stored, outside opening not closed to prevent entrance of insects or rodents.

One Stop: Food score 90; Retail score 91 — Food violations: Hand washing facilities not supplied/accessible, food not stored covered in freezers, improper hot holding temperature on multiple items (voluntarily discarded), toxic substances not properly labeled, wiping cloths in use in sanitizer bucket, mops not stored properly, food contact surfaces not easily cleanable. Retail violations: Hand washing facilities not supplied/accessible, over the counter medication expired, mold like substance in large ice machine, bulk ice sold at store without proper label, mops not stored properly, single use/single service articles not stored properly.

Ready Mart: Food score 98; Retail score 98 — Food violations: Missing date markings on items in deli cooler Retail violations: Bulk ice sold not properly labeled, single service/single use articles not stored properly.

Bluegrass Bar-b-eque: Food score 94 — Violations: Missing date marking on items, thermometers missing or difficult to locate, in use wiping cloths not properly stored, in use utensils not properly stored, non-food contact surface (free standing fans) with debris.

Opa’s Market: Food score 97; Retail score 98 — Violations: Food: Missing proper date marking on multiple items, mops not stored properly, sanitizer bucket too weak. Retail: Over the counter medicine expired.

K’s Café: Food score 98 — Violations: In use utensils not properly stored, single use/single service articles not properly stored.

Redneck Ranch Restaurant: Food score 90 — Violations: Food (multiple items) with out-of-date tags, date marking missing on multiple items in cooler, thermometer missing in multiple areas, in use wiping cloths not stored properly, in use utensils not properly stored, food/non-food contact surfaces not easily cleanable, fans with debris in food areas.

Twin Lakes Ice Creamery:  Food score 96 — Violations: Hair restraints not in use, in use utensils not properly stored, single use/single service articles not properly stored, cracked tile, daylight not around exterior door.

Matia’s Pizza & Wings: Food score 95 — Violations: Handwashing facility not supplied, missing date marking on multiple items, thermometers missing in multiple coolers/freezers, wiping cloths not properly stored.

Parkway Marathon: Food score 92; Retail score 96 — Food violations: No person in charge present at time of inspection, missing thermometers in refrigerator in deli area, contamination during storage and display, in use utensils not properly stored, single use/ single service articles not properly stored, non-food contact surfaces with debris, condensation in freezer in deli area. Retail violations: Contamination during storage (product on floor), single use/single service articles not properly stored, non-food contact surfaces with debris.

Dairy Barn: Food score 99 — Violations: Food not stored/covered.

Fastway #14 Valero: Food score 95; Retail score 95 — Food violations: Hot holding temperature out of range, food contact surface not easily cleanable, non-skid under items in deli case. Retail violations: Over the counter meds expired, product not stored properly to prevent contamination in walk-in cooler, single use/single service articles not properly stored, ceiling tiles in poor repair, multiple light covers with mold like substance.

Dinner Bell:  Food score 99; Retail score 100 — Violations: Food without covering in refrigerator.

Leitchfield Aquatic Concession (follow-up): Food Score 92 — Violations: Hand wash facilities not properly stocked, food not protected during prep from insects, thermometers not present or readily accessible in refrigerator or freezer, contamination during storage for food prep, hair restraints not worn, in use utensils not properly stored to protect from debris and insects.
